1. (a) A breach in the security or information relating to electronic poll books.
2. (b) An attempted breach in the security or information relating to electronic poll books.
3. (c) A defect in any of the electronic poll books.
4. (d) A failure in any of the electronic poll books.
5. (e) A fault in any of the electronic poll books.

County officials and electronic poll book vendors shall notify the Secretary of State within 24 hours upon discovery of any of the following:
