1. (a) Any person claiming to be aggrieved by an employment practice made unlawful by the FEHA may file a pre-complaint inquiry with the department by telephone, in person, by mail, or on the department's Web site at www.dfeh.ca.gov that shall describe the unlawful conduct alleged and include the following, on a form prescribed by the department:

   1. (1) complainant's name and, where available, address, telephone number and e-mail address;
   2. (2) respondent's name, address and, where available, telephone number and e-mail address. If applicable, the job title and/or capacity in which the respondent and/or co-respondent is being named also shall be included;
   3. (3) a description of the alleged act or acts of discrimination, harassment or retaliation;
   4. (4) the date of the most recent alleged act of discrimination, harassment or retaliation; and
   5. (5) each protected basis upon which the alleged discrimination or harassment was based.
2. (b) After submission of the pre-complaint inquiry, the department will initiate the investigative process as provided in Section 10007 of these regulations.
