(a) To obtain aid under this Article, the following must be submitted:

1. (1) Application on forms provided by the Board.
2. (2) Layout plans showing site preparation and stationing of portables in question. Such plans should clearly identify the location of such site.
3. (3) A statement by the district specifying how any costs auxiliary to the buildings themselves, but necessary for their use, are to be met.
4. (4) A statement of the estimated costs of all of the facilities requested by the district. The estimate should be made by a person currently employed or retained by the district deemed competent for this purpose by profession, training, or experience.
