1. (a) Law Enforcement Agency Endorsement issued pursuant to Section 236.5 of the Penal Code;
2. (b) Human trafficking caseworker as identified in Section 1038.2 of the Evidence Code, has attested by affidavit that the individual was a victim of human trafficking;
3. (c) Certification or eligibility letter from a government agency for a U Visa that is specifically based on a finding that the applicant was a victim of human trafficking, a T Visa, or other government benefits as a result of human trafficking; or
4. (d) Medical records documenting injuries consistent with human trafficking. Medical or mental health records alone shall not be sufficient evidence that the qualifying crime of human trafficking occurred.
