1. (a) An applicant must prove all of the following:

   1. (1) the nature of the crime committed by the apprehended criminal or prevented by the action of the applicant; or

      1. (A) the nature of the action of the applicant in rescuing a person in immediate danger of injury or death as a result of fire, drowning, or other catastrophe; and
      2. (B) the circumstances involved;

   2. (2) the applicant's action substantially and materially contributed to:

      1. (A) the apprehension of a criminal;
      2. (B) the prevention of a crime; or
      3. (C) the rescue of a person in immediate danger of injury or death due to fire, drowning, or other catastrophe;

   3. (3) as a direct consequence of subdivision (a)(2), the applicant suffered:

      1. (A) personal injury;
      2. (B) property damage; or
      3. (C) death; and
   4. (4) the extent of the injury or damage under subdivision (a)(3) for which applicant was not compensated from any other source.
