1. (a) The name and address of the person in whose favor the warrant is to be drawn.
2. (b) The items briefly and clearly stated in chronological order, together with all the charges for the period covered by the schedule.
3. (c) All entries shall be carried to the dollars and cents column direct.
4. (d) No items shall be interlined.

In addition to the information required under Section 622 every schedule of bills filed shall show:
