1. (a) An application for intent to contract submitted in response to the public notice required by Section 56078 shall include, but not be limited to:

   1. (1) The name, address and telephone number of the prospective FHA administrator;
   2. (2) The prospective FHA's program design as specified in Section 56084;
   3. (3) The signature of the prospective FHA administrator and the date on which the application was signed; and
   4. (4) Any additional format or procedural requirements deemed necessary by the regional center.
