1. (a) SLS vendors shall maintain, and provide access to, records pursuant to all applicable requirements of Title 17, Sections 50603, 50604, and 50605.

2. (b) In addition to the requirements noted in (a), all SLS vendors shall maintain and provide access to all records relating to service design, service delivery, and employee service time records. These records shall include the following:

   1. (1) Time sheets;
   2. (2) Payroll records;
   3. (3) Accounting records;
   4. (4) Training records;
   5. (5) Service evaluations;
   6. (6) Internal grievance procedure records;

   7. (7) Historical data documenting the actual delivery of service to consumers for which the SLS vendor has claimed payment, including the:

      1. (A) Identification of the vendor by unique identifier;
      2. (B) Location of the service;
      3. (C) Description of the service; and
      4. (D) Inclusive dates of the service; and

   8. (8) Other records as required by:

      1. (A) The terms of the contract; and
      2. (B) The regional center, for the purpose of conducting the evaluations specified in Sections 58671(c).
