1. (a) The vendor shall submit the following projected vendor information, for all income related to the transportation service being provided to the regional center on the Form DS2 (7/91), entitled Transportation Cost Statement:

   1. (1) The name of each vendor income source;
   2. (2) The duration of projected vendor income from each source; and
   3. (3) The total amount of projected vendor income from each source.

2. (b) Vendor income shall not include:

   1. (1) Donations;
   2. (2) Income received form the Department or regional centers.
