1. (a) The vendor shall maintain, and keep confidential, the consumer information received from the regional center pursuant to Section 58511.

2. (b) The vendor shall release the following consumer information only to the driver and only when necessary to protect the health and/or safety of the consumer, driver, transportation aide or other passengers:

   1. (1) The name, address, and telephone number of the consumer, authorized consumer representative and/or physician;
   2. (2) The consumer's disabilities, medication needs, allergies, and/or other conditions which may affect the vendor's provision of transportation services; and
   3. (3) Additional consumer information determined necessary by the regional center.
