1. (a) The Department considers an application for a certificate in nuclear medicine technology acceptable if all of the following conditions have been met:

   1. (1) Application is made on a Nuclear Medicine Technology Certificate Application form DHS 8435 (7/87) furnished by the Department.
   2. (2) Fee is paid pursuant to Section 30535.
   3. (3) Documentation provided with the application establishes that the applicant's basic education, laboratory instruction and clinical experience meet at least the competency criteria set forth in Section 30520.
