1. (a) The licensee's name and license number as shown on the specific license.
2. (b) The nature and scope of the request.
3. (c) The reasons for the request and supporting justifications including any documents relied upon.
4. (d) If the request proposes to increase the maximum possession limit specified on the license, the request shall include the fee specified in section 30231(c).
