1. (a) Is a physician, or has a master's degree in genetic counseling and is board eligible or board certified by the American Board of Genetic Counseling, or is certified by the Department of Public Health as having successfully completed an approved sickle cell counselor training program; and
2. (b) Provides face-to-face information on the medical, social, and genetic consequences of sickle cell disease and trait.

“Sickle cell counselor” means a person who:
