1. (a) Every laboratory performing forensic alcohol analysis shall provide the Department with the following within 90 days of any change to the information listed in 1-4 below:

   1. (1) A statement of intent to perform or stop performing alcohol analysis, to include fluid analysis, breath analysis, or both;
   2. (2) The laboratory's current address, as well as the name, address, and phone number of the laboratory's point of contact;
   3. (3) A list of current laboratory personnel qualified to do forensic alcohol analysis; and
   4. (4) A list of current instruments used by laboratory personnel for alcohol analysis.
2. (b) Forensic alcohol analysis shall be performed only by persons who meet the qualifications set forth in Section 1216.1 for forensic alcohol analysts.
