Electronic Only Medical and Social Information Collected for Certificate of Live Birth.

- (a) In addition to the items collected pursuant to Health and Safety Code sections 102425 and 102426, the unique ten (10) digit newborn screening number from the completed California Newborn Screening Test Request Form (CDPH Form 4409) shall be captured on the electronic copy only of the certificate of live birth. The ten (10) digit newborn screening number must not be transcribed onto the actual hard copy of the certificate of live birth.
- (b) The information provided pursuant to subdivision (a) shall be kept confidential pursuant to Health and Safety Code sections 102430 and 102447 and access is only permitted as specified by Health and Safety Code section 102430.
