1. (a) A statement that describes with particularity the nature of the violation, including a reference to the specific provision that has been violated;
2. (b) A statement that the suspension is immediately stayed and the license is placed on probation for one year;
3. (c) The effective dates of probation; and
4. (d) A description of the appeal process.
