Cal. Code Regs. tit. 16, § 310.1
Any licensee shall be entitled to a replacement license if their original license is lost, stolen or mangled, or upon written request and legal documentation of name change. Each request for a replacement license must be accompanied by a nonrefundable fee of $71, and signed written statement as to the circumstances of loss of said license, or the return to the Board office of the mangled license.
