1. (a) Reports of training shall be submitted to the Board, on forms prescribed by the Board, by the administrator-in-training and the preceptor within 10 days after the completion of 25 percent, 50 percent, 75 percent and 100 percent of the prescribed number of hours. The administrator-in-training and the preceptor shall be equally responsible for submitting reports on a timely basis.

2. (b) The administrator-in-training shall give written notice to the board:

   1. (1) If the administrator-in-training is to be absent, or has been absent, from training for more than 20 consecutive working days or a cumulative total of more than 40 working days.
   2. (2) If the administrator-in-training desires to change preceptors and/or facilities.
3. (c) If a training program is terminated before the administrator-in-training has completed the required number of hours, the administrator-in-training and the preceptor shall each notify the Board in writing within 10 days. Each shall state the reason or reasons the program was terminated.
4. (d) No material change in an approved training program shall be made unless approved by the Board. If a preceptor desires to make a material change in the amount or kind of training to be provided, he or she shall submit to the Board a written request for approval.
