1. (a) An application shall be deemed to have been abandoned and the application fee forfeited when:

   1. (1) The applicant fails to complete the application within two years after it is originally submitted;
   2. (2) The applicant fails to complete the application within two years after the last notification of deficiency; or
   3. (3) The applicant without good cause fails to take the examination within one year after being scheduled.
2. (b) An application submitted subsequent to the abandonment of a former application shall be treated as a new application.
