1. (a) The certification course provider in blood withdrawal for licensed vocational nurses must submit to the Board a copy of the certificate issued to the licensee, entitled, “Certificate of Completion Blood Withdrawal For Licensed Vocational Nurses” (Form 55ED-2; 09/05), hereby incorporated by reference, which shall include:

   1. (1) Course title;
   2. (2) Date of course completion;
   3. (3) Licensee's name, address, telephone number and vocational nurse license number;
   4. (4) Code number issued by the Board; and
   5. (5) Number of course hours, specified as theory and clinical hours.
2. (b) The certification course provider shall maintain a list of all licensed vocational nurses who have completed the blood withdrawal certification course within the preceding four years.
3. (c) A licensed vocational nurse is deemed to be Board-certified in blood withdrawal upon written notification from the Board.
