1. (a) The board may issue a duplicate certificate to a certificate holder upon the written request therefor by such certificate holder. Such request shall be accompanied by the required fee and an affidavit or declaration in a form provided by the board showing to its satisfaction the loss, mutilation, or destruction of the certificate holder's original certificate.
2. (b) A certificate holder who changes their name according to law may request the board to issue a duplicate certificate. Such request shall be accompanied by evidence thereof satisfactory to the board and payment of the required fee. The board shall thereafter issue a duplicate certificate setting forth the name of the certificate holder as changed.
