1. (a) Failure to report to the board within 30 days any disciplinary action taken by any public agency in any state or territory or any licensing entity or authority of this state or another state or territory, any agency of the federal government or United States military, or a foreign country.
2. (b) Failure or refusal to comply with a court order, issued in the enforcement of a subpoena, mandating the release of records to the board.
