1. (a) An application shall be deemed to have been abandoned and the application fee forfeited when the applicant:

   1. (1) Fails, without good cause, to appear for examination within one year or two subsequent examinations, whichever first occurs, after notification by the board; or
   2. (2) Fails to submit the initial license fee within two years after notification by the board.
2. (b) An applicant whose application has been deemed abandoned may again become eligible for examination or re-examination upon filing a new application and paying the application fee.
