1. (a) Name of licensee and license number or other identification number;
2. (b) Course title;
3. (c) Provider name and address;
4. (d) Board-recognized approval agency name, or a statement that the provider is an entity recognized by the board to provide continuing education pursuant to section 1887.4.3;
5. (e) Date of course;
6. (f) Number of hours of continuing education credit; and
7. (g) Signature of course instructor, provider, or provider designee.

Upon completion of a course, a provider shall issue a record of course completion to a licensee (e.g., letter verifying attendance, certificate, grade slip, transcript) containing the following information:
