1. (a) Successful completion of the program designated by the committee.
2. (b) Failure to comply with the rehabilitation program designated by the committee.
3. (c) Failure to comply with any of the requirements set forth in Section 1447.
4. (d) Failure to substantially benefit from participation in the program.
5. (e) Receipt of information by the board which, after investigation, indicates the participant may have violated a provision of the laws governing the practice of nursing, Chapter 6 (commencing with Section 2700) of Division 2 of the Code, excluding Section 2762.

The committee may terminate a nurse's participation in the program for any of the following reasons:
