1. (a) A person may petition for reinstatement of a license under the provisions of Section 11522 of the Government Code by:

   1. (1) Obtaining the appropriate forms from the board office in Sacramento and
   2. (2) Submitting the required form of petition and supplementary documentation to the board at least forty-five (45) days in advance of the date on which the person desires to be heard, as directed in the written instructions which accompany the forms.
2. (b) The burden of proving rehabilitation is upon the petitioner. When considering a petition for reinstatement of a license, the board shall evaluate and consider evidence of rehabilitation submitted by the petitioner, using those criteria specified in Section 1445 of this article.
