1. (a) Gravity of the violation.
2. (b) History of previous violations of the same or similar nature.
3. (c) Length of time that has passed since the date of the violation.
4. (d) Consequences of the violation, including potential or actual patient harm.
5. (e) The good or bad faith exhibited by the cited individual.
6. (f) Evidence that the violation was willful.
7. (g) The extent to which the individual cooperated with the board's investigation.
8. (h) The extent to which the individual has remediated any knowledge and/or skills deficiencies which could have injured a patient.
9. (i) Any other mitigating or aggravating factors.
