1. (a) Each person or approved program holding a license or approval and each person or program who has an application on file with the board shall notify the board at its office of any and all changes of mailing address within thirty (30) calendar days after each change, giving both the old and new address.
2. (b) If an address reported to the board is a post office box, the licensee shall also provide the board with a street address, but he or she may request that the second address not be disclosed to the public.
