1. (a) The physician has failed to comply with the diversion agreement, including but not limited to, failure to comply with the prescribed monitoring or treatment regimen, use of alcohol or other unauthorized drug; or refusal to stop practice when directed to do so by the committee.
2. (b) Any cause for denial of an applicant in Section 1357.4.
3. (c) The physician has failed to comply with any of the requirements set forth in Section 1357.1.
4. (d) The committee recommends that the physician will not benefit from further participation in or has not substantially benefited from participation in the program or that the physician's continued participation in the program creates too great a risk to the public health, safety or welfare.

The program manager may terminate a physician's participation in the program for any of the following reasons:
