1. (a) Applications for a fictitious name permit shall be submitted on a form provided by the division containing such information as is required therein.
2. (b) The application shall be signed by a licensed person who is a member of the group or clinic or by an officer of the professional corporation, as the case may be, requesting the use of such name.
