1. (a) Nature and severity of the violation.
2. (b) Length of time that has passed since the date of the violation.
3. (c) Consequences of the violation, including potential or actual patient harm.
4. (d) History of previous violations of the same or similar nature.
5. (e) Evidence that the violation was willful.
6. (f) Gravity of the violation.
7. (g) The extent to which the cited person has remediated any knowledge or skill deficiencies, which could have injured a patient.
