1. (a) The person shall be at least 21 years of age;
2. (b) The person shall have a documented history of opiate use of at least five years;
3. (c) The person shall have had at least one prior medically supervised detoxification.

The Board shall decide whether to approve an outpatient's participation in a methadone maintenance program on a case-by-case basis. The following are the guidelines which the Board shall use in its consideration:
