1. (a) Within 45 calendar days after the end of each quarter, the county or city may be required to submit verification of training completed during the quarter pursuant to the training plan.

2. (b) The quarterly report shall contain information required by the Board, and shall include but not be limited to:

   1. (1) The number of staff trained, certified courses completed, and hours completed.
   2. (2) The total subvention expenditures for costs necessary to pay staff who attend training on regularly scheduled time off or who replace employees attending training.
   3. (3) The total subvention expenditures for the entire training program.
   4. (4) Significant changes, problem areas, and any other significant data or observations regarding the program or deviations from the training plan.
