1. (a) A brief description of the proposed activity;
2. (b) all substantial, adverse environmental impacts that the proposed activity may cause;
3. (c) all irreversible environmental impacts that the proposed activity may cause;
4. (d) any feasible mitigation measures that would reduce such substantial adverse environmental impacts;
5. (e) any feasible alternatives, including design alternatives, to the proposed project that would reduce such substantial adverse environmental impacts; and
6. (f) such other information that the Executive Director believes appropriate.

An assessment of the substantial adverse impacts of a proposed activity shall contain a summary of the following:
