1. (a) A copy of the unit agreement and any applicable unit operating agreement.
2. (b) A report, accompanied by appropriate data, specifying in detail the nature of the disagreement.
3. (c) A recommended resolution of the disagreement, accompanied by supporting data and calculations.
4. (d) The names and addresses of all working interest owners in the unit, and all persons listed in the records of the county tax assessor as having an interest in the lands included in the unit area.

A petition requesting review and decision by the Supervisor of a disagreement with respect to unit operations and each copy shall contain:
