1. (a) The names of all parties involved in the dispute and, if any, their representatives, including attorneys.
2. (b) The physical addresses, electronic mail addresses, and telephone numbers of the parties or their representatives, including attorneys.
3. (c) A brief statement of the nature of the dispute.
4. (d) Documentation related to the dispute, including records and arguments submitted to the State Agency and/or Museum.
5. (e) Relief requested by the parties.

Upon issuance of a Commission notification in response to a written notice of dispute described in section 31005(a), each party shall have 45 days to submit the following information to the Commission:
