1. (a) A manufacturer authorized to operate an SAE International's Level 4 or 5 autonomous vehicle on public roads in deployment, or a manufacturer providing an automated driving system service, after receipt of notice of a vehicle immobilization shall prepare and submit to the department a quarterly report summarizing occurrences. Manufacturers that provide privately owned vehicles for non-commercial use shall report each instance of an immobilization, but are not required to provide the date and time of the occurrence, the longitude and latitude, or vehicle identification number.

   Reports shall be provided on the following dates: March 31^st; June 30^th; September 30^th; December 31^st. The first report is due for the first full calendar quarter after the effective date of the regulations. The report shall be submitted using the electronic Vehicle Immobilization Reporting Template (Rev. 2/2025), provided by the department, which is hereby incorporated by reference. The manufacturer shall submit the report electronically in .csv format via the department's web page or portal. If no vehicle immobilization occurred during the reporting period, the manufacturer shall report such to the department.

2. (b) The report shall include the requirements described in Section 227.58 of Article 3.7.
