1. (a) The application for a Certificate of Self-Insurance, form SR44 (REV. 6/01) shall be accompanied by an audited financial statement for the three-year period immediately preceding the date of application. The financial statement shall include an opinion of the net worth of the applicant rendered by an independent certified public accountant.

   1. (1) This opinion shall apply to the net worth of the applicant for the date and time when the financial statement is signed.
2. (b) When an applicant has not been in existence for three years prior to the date of application, the department may accept a financial statement covering the period the applicant has been in existence.
