Cal. Code Regs. tit. 11, § 4401 – Uniform CCW License Design Standards.

(a) The uniform CCW license mandated by Penal Code section 26175, subdivision (a)(3)(A), shall be 2.125 inches in height and 3.375 inches in width. The uniform CCW license shall be laminated or printed on a hard card using form BOF 4501.
(b) The front of the uniform CCW license shall be in the following format:
(c) Licensing authorities may add their seal to the top left corner of the front of the uniform CCW license and must select green or blue for the banner and text. No other modifications are allowed. For “DL/ID,” indicate the number of the licensee's California driver's license or identification card issued by the California Department of Motor Vehicles. For non-California residents, indicate the number of the licensee's driver's license or identification card issued by the licensee's state of residence. For “Lic. Issued,” indicate “Initial” or “Subsequent.” For “Type,” indicate the type of license issued as it relates to Penal Code section 26220 (i.e., Standard, Nonresident Standard, Employment, Judicial, Nonresident Judicial, Custodial, Reserve PO [peace officer]). For “Issued by,” include the signature of the sheriff, chief of police, or other head of agency. The sheriff, chief of police, or other head of agency may authorize an individual employed by the licensing authority to sign in their place.
(d) The back of the uniform CCW license shall be in the following format:
(e) If more space is needed to list additional weapons authorized to be carried, they shall be listed under “Restrictions, Comments, and Additional Firearm information (if any).”

History

1. New section filed 3-4-2024 as an emergency; operative 3-4-2024 (Register 2024, No. 10). Pursuant to Penal Code section 26225(d) this action is a deemed emergency and exempt from OAL review. A Certificate of Compliance must be submitted to OAL by 1-1-2026 or the emergency language will be repealed by operation of law on the following day.
