1. (a) The name of the organization;
2. (b) If the current officer address is to be changed, the street address of the new officer address in accordance with section 12626 of the Government Code;
3. (c) If the current responsible officer is to be changed, the name of the new responsible officer and the new officer's written consent accepting the responsibility of the responsible officer.

An organization may change its responsible officer or responsible officer address by delivering to the Attorney General, within thirty days of the change, a statement of change that sets forth:
