1. (a) The course provider shall maintain a student record for each participant for a period of at least five years from completion of the course. The record shall include, at a minimum, the following information:

   1. (1) The Bureau's accreditation number for the offering;
   2. (2) Name and address of each participant;
   3. (3) Course title and hours of credit approved;
   4. (4) Date(s) of course and date course is completed;
   5. (5) Name and address of course provider;
   6. (6) Identity of verifier of attendance; and
   7. (7) Course grade or other evidence of satisfactory completion.

2. (b) The course provider shall maintain business records for a period of at least five years. The records shall include, at a minimum, the following information:

   1. (1) Business and mailing address of course provider as on file with the Bureau;
   2. (2) Written records of attendance and grades for each student; and
   3. (3) The principal place of business or the mailing address at which each of the accredited basic education and continuing education instructors can be contacted.
3. (c) Upon receipt of a request from the Chief, the records specified in this section shall be submitted to the Bureau within 30 days.
4. (d) Whenever there is a change in the business or mailing address of the course provider, the course provider shall notify the Chief in writing within 10 days of the change.
