1. (a) Applicants who fail an examination may apply for re-examination by submitting the following:

   1. (1) Request for Re-Examination, Form REA 3007 (Rev. 5/8/00), which is herein incorporated by reference; and
   2. (2) All applicable fees pursuant to Section 3582.
2. (b) Applicants shall not be permitted to take any subsequent examination without a scheduled time and payment of applicable fees.
3. (c) Applicants shall not be allowed more than 5 attempts to pass the examination in any 12-month period.
