1. (a) Upon request by the Bureau, the applicant shall submit course description documentation.

2. (b) Course description documentation shall include but not be limited to:

   1. (1) Course syllabus;
   2. (2) Course outline;
   3. (3) Copies of the table of contents or copies of the index pages of the textbook used during the course; and

   4. (4) Letters of verification from instructors, signed under penalty of perjury, verifying the following:

      1. (A) Name of school;
      2. (B) Name of student;
      3. (C) Name of course;
      4. (D) Date course was taken;
      5. (E) Course grade or other evidence of satisfactory completion;
      6. (F) Topic areas taught in the course; and
      7. (G) Signature and date of instructor or other appropriate school official.
