1. (a) If authorized by the board, the program shall pay an insurance agent as defined in section 31 of the Insurance Code or broker as defined in section 33 of the Insurance Code for assisting an individual in completing the application form, if the following conditions are met:

   1. (1) The individual is enrolled as a result of the application;
   2. (2) The agent or broker requests such payment in writing; and
   3. (3) Such request accompanies the application and includes the name, license number, tax identification number or social security number, telephone number, and fax number (if available), signature, and address of the agent or broker.
2. (b) The amount of such payment shall be $100.00.
