Appendix B
Disclosure Categories
Designated positions in Category 1 shall report the following:
• Investments and business positions in business entities and sources of income (including receipt of gifts, loans, and travel payments) if the business entity or source which, within the preceding 24 months, has been subject to regulation, licensing, enforcement, or oversight by or under the jurisdiction of the Department.
Designated positions in Category 2 shall report the following:
• Investments and business positions in business entities and sources of income (including receipt of gifts, loans, and travel payments) if the business entity or source provides leased facilities, products, equipment, vehicles, machinery or services (including training or consulting services) of the type utilized by the Department.
Designated positions in Category 3 shall report the following:
• Investments and business positions in business entities and sources of income (including receipt of gifts, loans, and travel payments) if the business entity or source is required to be licensed by the Department, or that provides education or training, which, within the preceding 24 months, have been required by the Department to qualify for or maintain a license.
Designated positions in Category 4 shall report the following:
• Investments and business positions in business entities and sources of income (including receipt of gifts, loans, and travel payments) if the business entity or source has been the subject of investigation, or reviewed by the designated position during the past 12 months.
Designated positions in Category 5 shall report the following:
• Investments and business positions in business entities and sources of income (including receipt of gifts, loans, and travel payments) if the business entity or source provides leased facilities, products, equipment, vehicles, machinery or services (including training or consulting services) of the type utilized by the position's bureau, branch, division, or area of assignment.
Designated positions in Category 6 shall report the following:
• Investments and business positions in business entities and sources of income (including receipt of gifts, loans and travel payments) if the business entity or source provides information technology or telecommunications goods, products or services including computer hardware or software companies, computer consultant services, IT training companies, data processing firms and media services to the Department, including California Municipal Award Schedule contractors.
Designated positions in Category 7 shall report the following:
• Investments and business positions in business entities and sources of income (except for income received solely from the sales of insurance products or solely for adjusting insurance claims) (including receipt of gifts, loans, and travel payments) if the business entity or source, within the preceding 24 months, has been approved by the Department as education providers.
• Investments and business positions in business entities and sources of income (except for income received solely from the sales of insurance products or solely for adjusting insurance claims) (including receipt of gifts, loans, and travel payments)if the business entity or source is an insurance agent, broker, adjusteror bail agent who is subject to the permit or licensing authority of, or have an application for a license or permit pending before the Department.
Designated positions in Category 8 shall report the following:
• Investments and business positions in business entities and sources of income (including receipt of gifts, loans, and travel payments) if the business entity or source is a workers' compensation insurers.
Designated positions in Category 9 shall report the following:
• Interests in real property located in the state of California.
