1. U. C. D. policies and forms shall be submitted to the commissioner for filing with and approval by the commissioner in conformity with the requirements of Article 1 of this subchapter applicable to other disability policies and forms, except that in the submission of final copies for approval, each form shall be submitted in triplicate. In addition, one copy should be submitted to the Department of Employment Development, Division of Disability and Hospital Benefits, Sacramento, California.
