Submissions of additional or revised insert pages for use with previously approved or filed documents must include a cover letter stating the California Department of Insurance file numbers for the previous filings, the filing form numbers, a description of the types of documents with which the insert pages will be used and the dates of the Commissioner's action on those documents.
