If the applicant will offer or provide debt settlement services under the registration, the applicant shall as part of the registration application submit directly to the Commissioner at [email protected] copies of sample periodic account or activity statements used by the applicant to provide debt settlement services to California residents, if the applicant provides statements to California residents.
