1. (a) In case the licensee obtained any new payment instrument or travelers check license during the quarter from a governmental agency within the United States, provide the following information with respect to each such license:

   1. (1) Name, address, and, if available, telephone number of the governmental agency by which the license was issued.
   2. (2) Business which the applicant is authorized to transact under the license.
   3. (3) Any material conditions to which the license is subject.
2. (b) In case any application by the licensee for a new payment instrument or travelers check license was denied during the quarter by a governmental agency within the United States, provide a copy of the order, decision, or other instrument by which such application was denied.

3. (c) In case any payment instrument or travelers check license of the licensee which was issued by governmental agency within the United States was suspended, revoked, voluntarily surrendered, or otherwise terminated during the quarter or in case any payment instrument or travelers check license of the licensee which was issued by a governmental agency outside the United States was suspended, revoked, or otherwise involuntarily terminated (other than for nonpayment of fees) during the quarter, provide the following information:

   1. (1) State the name, address, and, if available, telephone number of the governmental agency by which the license was issued.
   2. (2) Describe the business which the applicant was authorized to transact under the license.
   3. (3) State the date on which the license was terminated.
   4. (4) State the reasons why the license was terminated.
