1. (a) An application by a licensee for approval under Financial Code Section 2100 of the form of receipt used by the licensee or by its agent shall contain the following:

   1. (1) Provide a sample form of receipt.
   2. (2) Provide a certification by an officer of the applicant which certifies that the sample form of receipt is the receipt form used by the applicant and its agents.
   3. (3) Provide a translator's certificate, if applicable, in accordance with Section 5.3002, Title 10, California Code of Regulations.
